Here is Important Information you can use as a checklist for completing the application, what needs to be included, and how to submit your completed package to us for review.

The DEADLINE to send back the application is: June 30 of each year.
DO NOT wait until this date to mail in your completed application packet or you will miss the deadline.
Include TWO letters of recommendation. You may ask your care provider (or a past care provider), your CPS, ILP, or foster family/group home agency social worker, a counselor, a teacher, and/or an employer.
Very Important: Make a copy of the completed application and each letter of reference for yourself. (You should make a copy every time you send important information through the mail, in case it gets lost!)
Attach a copy of your HS transcript (it does not have to be the “official” copy), along with proof that you have filed your FAFSA for the school year. For example, your student aid report or FAFSA confirmation page will complete this requirement. You will also be expected to apply for the Chafee Grant at www.csac.chafee.ca.gov and show proof of having done this.
Fill out all questions neatly and completely. Attach additional pages if necessary.
You may print or type the information. You might want to make a copy first, and then fill out the original. Neatness and how completely you filled out the application will be considered when we decide which applications will go forward to the scholarship committee.
Don’t forget to sign your application!
